Advance Care Planning: Supporting Person-Centric Healthcare Decisions
DESCRIPTION:
Advance care planning empowers individuals to make informed healthcare decisions before a medical crisis occurs. This session provides an overview of advance directives, including the role of the healthcare agent, the differences between electronic and written directives, and options for life-sustaining treatment. Participants will learn how advance care planning supports individual autonomy, aligns care with personal values and goals, and reduces family conflict.
This session also explores practical strategies for initiating advance care planning conversations, addressing common misconceptions, and connecting individuals with appropriate legal, medical, and community resources. Participants will leave better prepared to support clients through informed discussions while practicing within their professional scope.

Valoria Walker is the Founder of Doula by Destiny, an organization dedicated to partnering with healthcare providers, community organizations, and families to advance compassionate end-of-life care through education, advocacy, and advance care planning.

Maryland Gerontological Association offers One Category 1 Social Work CEU Credit and One CMC Contact Hour to those that attend a live session and complete the evaluation by the posted deadline.
To claim your CEU Credit:
- Attend the session in its entirety.
- Complete and return the mandatory program evaluation by the posted deadline.
Within 48 hours of the posted deadline, CEU certificates will be emailed to participants who have completed the evaluation. It is the responsibility of the participant to maintain accurate records of their credit hours.
